Can You Get Dental Implants if You Smoke or Have Diabetes?

Can You Get Dental Implants if You Smoke or Have Diabetes?

October 21, 2026

Often, yes, but not automatically. Smoking and diabetes do not rule out dental implants on their own. They do affect how the gum and jawbone heal and how well an implant is looked after over the years, so dentists treat them as risk factors that need to be understood before treatment is planned.

What matters for diabetes is usually how well it is controlled, and what matters for smoking is whether you smoke now, how much, and whether you are willing to stop around the time of treatment. Whether implants are sensible for you is a decision for your implant dentist and your own doctor to reach together, after an examination. This post explains what the named health authorities say, what the consultation involves, and what can improve the outlook.

Key takeaways

  • Smoking and poorly controlled diabetes are both listed by NHS hospital trusts as factors that raise the chance of implant problems. Neither one makes the decision on its own.
  • For diabetes, how well it is managed is what your dentist and doctor will focus on, rather than the diagnosis alone.
  • Stopping smoking before and after treatment is advised by NHS dental teams, and free NHS support exists to help.
  • Your dentist will ask about your health, medicines, smoking and gum health, and may need a letter or advice from your doctor.
  • If implants are not advised, or not yet advised, there are other ways to replace missing teeth.

Can smokers and people with diabetes have dental implants?

A dental implant is a small post placed in the jawbone that fuses with the bone over time and holds a crown, bridge or denture. For that to happen, the gum has to heal, the bone has to bond to the implant, and the tissues around it have to stay healthy for years. Bone matters here, and our post asks is it ever too late to get a dental implant.

Smoking and diabetes can both interfere with that process. According to Leeds Teaching Hospitals NHS Trust, dental implants are not the right option for everyone with missing teeth, and suitability depends on factors including overall health and smoking status. The same trust lists a smoking habit and poorly controlled diabetes among the things that make the chance of late implant failure greater.

That wording is worth reading closely. It speaks of a greater chance, not a certainty, and it says poorly controlled diabetes, not diabetes. Plenty of people who smoke or live with diabetes are assessed carefully and go on to have implants, and others are advised to wait or to choose a different option. Only an examination can show which applies to you.

How smoking affects healing and implant risk

Smoking is one of the most consistently named risk factors for implant problems. Guy's and St Thomas' NHS Foundation Trust says it strongly recommends that patients try to stop smoking and remain a non smoker in the long term, and that this lowers the risk of some complications with dental implants. Cambridge University Hospitals NHS Foundation Trust describes smoking as a significant risk factor and advises stopping at least six months before starting implant treatment.

Why it matters for the gum and bone

Smoke irritates the gums and can reduce how well the mouth heals after surgery. An implant needs a settled, healthy gum around it and a period of undisturbed healing while the bone bonds to the implant. Anything that disrupts either stage raises the question of whether the implant will bond as planned.

Smoking also matters after the implant is in place. Gum inflammation and bone loss around an implant, known as peri implantitis, can develop over time. Leeds Teaching Hospitals notes that inflammation of the gum and bone loss may occur around an implant and can eventually lead to its loss. Our post on dental implant infection signs lists what to look out for.

What a published review found

A 2023 summary in Evidence Based Dentistry, by researchers at the Peninsula Dental School, University of Plymouth, looked at a meta analysis of 21 observational studies. It reported that smokers had a higher risk of peri implantitis than non smokers. Observational studies show an association rather than proving cause, and they cannot predict what will happen to one person, but they are consistent with the advice above.

Smoking and the weeks after surgery

The first days matter most. Our own aftercare advice lists smoking before the wound has healed as something to avoid. If you smoke, the plan the dentist suggests may include a period without smoking both before and after the procedure, and your dentist will explain what they advise in your case.

How diabetes affects healing and implant risk

Diabetes is a long term condition in which blood sugar levels can run high. When blood sugar is well managed, the mouth is more likely to behave in the same way as anyone else's. When it is not, healing can be slower and the gums can be more prone to infection and inflammation.

West Hertfordshire Teaching Hospitals NHS Trust says that when you have diabetes your risk of gum disease is higher because of high blood glucose levels, and that looking after your teeth and gums should be a basic part of managing diabetes. The NHS also advises people with type 2 diabetes to see a dentist and dental hygienist regularly. Gum health matters for implants because the same tissues surround an implant.

Why control matters more than the label

Leeds Teaching Hospitals names poorly controlled diabetes, not diabetes, as a factor in late implant failure. The Evidence Based Dentistry summary reported that patients with diabetes appeared to have a higher risk of peri implantitis, and noted that its authors based this on observational studies rather than on a trial. In practice, a dentist will want to know how stable your blood sugar has been, who looks after your diabetes, and whether you have had recent gum problems or slow healing.

You do not need to work out your own risk. Your diabetes team or GP knows your recent readings and any complications, and the implant dentist knows the demands of the surgery. They are best placed to advise together.

What your dentist will ask at the consultation

An implant consultation is as much about your general health as about your teeth. Expect questions on the following, and bring what you can.

  • Medical history. Your conditions, including diabetes, heart or circulation problems and anything affecting healing.
  • Medicines. Guy's and St Thomas' advises telling your dental team about any problems with your health and any tablets or medicines you take. Bring a list rather than relying on memory.
  • Diabetes control. How it is managed, who looks after it, and how stable your recent readings have been. Your doctor or diabetes team may be asked for information.
  • Smoking and nicotine. What you use, how much, and your plans to stop. Be open about it, because the plan depends on it.
  • Gum health. Any bleeding, loose teeth or past gum treatment, because gum disease history is another factor Leeds Teaching Hospitals lists.
  • Bone and scans. The dentist will assess the jaw, often with a 3D scan, because the amount of bone also decides whether an implant is possible.
  • Your own goals. Whether you want a fixed or removable result, and how you feel about surgery and a longer timeline.

Our new patient registration form already asks whether you smoke and whether you have diabetes, alongside a wider medical history, so it helps to complete it carefully. The implant consultation is the point at which the dentist reviews all of this with you and explains the options.

After the assessment, the dentist may advise going ahead, going ahead after certain steps, waiting, or choosing another option. Each is a reasonable outcome, and the reasoning should be explained to you.

Ways to improve the outlook

Nothing here guarantees a result, but these are the steps NHS teams and dentists commonly point to.

If you smoke

  • Stop, or plan to stop, well before treatment. Cambridge University Hospitals advises at least six months before starting. Your dentist will tell you what they advise for you.
  • Use the free support available. The NHS Better Health site offers a free Personal Quit Plan and the free NHS Quit Smoking app, and a local stop smoking service can also help. Ask your doctor or pharmacist about support that suits you.
  • Stay smoke free afterwards. Guy's and St Thomas' recommends remaining a non smoker in the long term.

If you have diabetes

  • Keep your diabetes review appointments and share your recent results with your dentist if asked.
  • Ask your diabetes team or GP whether they have any advice for you before dental surgery, and let the dentist know what they say.
  • Look after your gums. See a dentist and hygienist regularly, and mention bleeding or sore gums early. Our gum disease treatment covers how gum problems are assessed and managed.

For everyone

  • Brush and clean between your teeth every day. Leeds Teaching Hospitals lists poor oral hygiene as another factor in late failure.
  • Follow the aftercare instructions you are given. Our implant aftercare advice includes written instructions and follow up visits after placement.
  • Keep attending check ups and hygiene visits, even once the implant is in place. Regular review lets the dentist spot gum or bone changes early.

How long does healing take?

Healing is not a fixed schedule, and it can be longer than average when health factors are involved. Leeds Teaching Hospitals says the gum usually heals within one to two weeks, with implants left to bond to the bone for around two to six months before use. Guy's and St Thomas' says that in total, treatment could take six to twelve months. Our implant healing stages outline what happens at each step.

If the bone needs building up first, it adds time. Our information on bone grafts explains why a graft might be needed and how long the bone is usually given to settle. Guy's and St Thomas' also notes that bone grafting can cause swelling, bruising and pain.

Alternatives if implants are not advised

Not being advised to have implants is not the end of the road. Leeds Teaching Hospitals lists several routes, and your dentist can talk through which suit your mouth.

  • Removable dentures. False teeth that are taken out for cleaning and at night. Our dentures services cover the main types.
  • Bridges. Artificial teeth held by neighbouring natural teeth, which depends on those teeth being suitable.
  • A different denture design, or in some cases leaving the space alone. Leeds Teaching Hospitals notes that in some cases it is better to accept the space where a tooth has been lost.
  • Delaying implants. You might plan implants for later, once smoking has stopped or diabetes is settled, if your dentist and doctor agree.

Some people choose an implant retained denture as a middle path. This is a removable denture that clips onto a small number of implants. Our implant retained dentures are described separately, and the same health questions apply.

When to see a dentist or seek urgent help

See your dentist if, after an implant, you notice bleeding or swelling around it, a bad taste, a loose implant or crown, or pain that is getting worse rather than better. Early review matters because infection around an implant is easier to deal with sooner. If you have diabetes, tell the dentist, as it can affect how infection is managed.

For urgent dental advice outside surgery hours, the NHS advises calling NHS 111. Go to A&E or call 999 if you have swelling that is spreading across your face or neck, or swelling that affects your breathing or swallowing, or bleeding that will not stop.

Talk to a dentist about your own situation

General information only goes so far, because the answer depends on your mouth, your health and your plans. If you are weighing up implants and have questions about smoking or diabetes, an examination lets a dentist look at your gums, bone and history, and explain the options in plain language. Bring your medicine list and any questions with you.

Dental implants, smoking and diabetes FAQs

Can I have dental implants if I smoke?

It depends on your assessment. Smoking is named by NHS hospital trusts as a risk factor, and teams commonly advise stopping before and after treatment. Some dentists will wait until you have stopped, and some will advise another option. Your dentist decides with you after examining your mouth.

Can I have dental implants if I have diabetes?

Many people with diabetes are assessed for implants. The focus is usually on how well the diabetes is controlled, and a dentist may ask your doctor or diabetes team for information. Leeds Teaching Hospitals names poorly controlled diabetes as a factor in late implant failure, so it is a conversation to have early.

Should I stop smoking before dental implants?

NHS dental teams recommend it. Cambridge University Hospitals advises stopping at least six months before starting treatment, and Guy's and St Thomas' recommends staying a non smoker long term. Your dentist will tell you the timing they advise for you.

Does diabetes make implants fail?

Not on its own. Poorly controlled diabetes is listed among the factors that raise the chance of late failure, and published reviews suggest a higher risk of peri implantitis in people with diabetes. These are risks across groups of people and cannot tell you what will happen in your own case.

What is peri implantitis?

It is inflammation of the gum and loss of bone around an implant, which can develop over time and, if not dealt with, may lead to the implant being lost. Good cleaning, regular check ups and early review of any symptoms help your dentist spot it sooner.

Do I need a letter from my doctor?

Sometimes. If your health could affect healing, the dentist may ask for information from your doctor or diabetes team before planning surgery. Telling your dentist about all your conditions and medicines at the start makes this easier.

What if I cannot stop smoking?

Be honest with your dentist, who can explain what the added risk means for you and which options remain. These could include delaying treatment while you get support, or choosing a bridge or denture instead. The NHS Better Health site has free support if you would like to try again.

Are there alternatives to dental implants?

Yes. Removable dentures and bridges are the usual alternatives, and in some cases leaving the gap is the better choice. Your dentist can explain the pros and cons of each for your mouth, and implants can be revisited later if your circumstances change.

Being very nervous about using dentists after a terrible experience when I was younger, I came to Maida Smiles and loved the calm patient demeanour of Pedro. You can clearly see Pedro and the team take pride in what they do and are perfectionists. The whole experience was great and I will never go to another dentist again.

Delighted with the outcome of my treatment by Pedro. Meticulous attention to detail explaining exactly how the procedure would be carried out from first consultation to completion backed up with supporting guidance notes. Clearly an expert in his field and someone who cares passionately about the outcomes for his patients. Highly recommended.

I loved my first visit to Maida Smile Dental Clinic. The staff member at reception was very warm, welcoming and professional. My dentist, Priscilla, and her assistant were fantastic. I have quite sensitive teeth near my gums and I can find the probing and investigation quite uncomfortable during a consultation, but this wasn’t the case at all. I didn’t experience discomfort and felt in very safe hands. This also applied to the hygienist work. I didn’t even have to ask Priscilla to avoid using the ultra sound cleaner on the sensitive areas of my teeth. I’m looking forward to going back, and that’s not often you can say that about going to the dentist!

Since I first had the luck to be recommended and book an appointment with Dr. Pedro Gutierres over 8 years ago, I have never trusted my dental health nor my family’s to anyone else. It’s very hard to find a dentist who ticks all the boxes and even goes beyond any expectations you may have, from service, sympathy, professionalism, and expertise, etc., 5 stars are not enough to rate him. I was so happy when he opened his own clinic and surrounded himself with the best professionals you can find, I cannot recommend Maida Smiles Dental Clinic enough.

“Pedro was so patient and professional throughout my treatment for my implants.. I chose to have my treatment with a local anaesthetic rather than any sedation and have a very bad gag reflex when anything touches the back of my throat. His patience throughout the experience was wonderful, completely putting me at ease and he also gave me a technique to help me in the future. I have seen him for my follow up and fully intend to book an annual visit in 2020. which is no small thing as I live 60 miles away.I can thoroughly recommend him for any treatment.”

Denise Gilbert

I was very nervous about having a dental implant but was immediately put at ease by Dr Pedro Gutierres. The procedure was explained in full and the care both during and after treatment has been of the highest standard. I received advice on how to look after my implant and have had no problems. I highly recommend.

Dawn Briggs

I had 7 implants installed by Dr Pedro Gutierres, including one which required a bone graft. In my 70 years I’ve had some bad experiences with dental treatment so it takes a lot for me to relax but, from my initial consultation, through to the end of the treatment, I had complete trust in Pedro’s expertise: preparations for the treatment and after-care were extremely thorough and fully explained, the treatment itself was surprisingly endurable (considering what was involved) and the implants have made a major difference to my life. If you are considering implants then I would whole-heartedly and sincerely recommend Pedro for this work.

Russ Hickman

I couldn’t recommend Maida Smiles enough: the care, attention to detail and the way Dentists and Nurses made sure I felt comfortable and safe throughout the treatments was incredible. Dr Pedro explained the treatments in detail to me and I have never felt under pressure to proceed… but I am very glad I did as I am happy and confident to smile again!

Mariana Pinheiro

I went to different clinics before, and nobody could really convince me until Maida Smiles did. The team here makes you comfortable , shows you different options and different budgets , and doesn't try to make you go only for one plan. I felt that the team was knowledgeable and wanted to help me , so that's why I decided to start

Soula

I feel like I've not just developed a professional relationship with my dentist and the staff... they become more like friends as well, because of the kind of compassion and sympathy that they give you during treatment.

I am always anxious when having to see the dentist and this was one of my first experiences with Maida Smiles. The dentist, Benedict Harrison was very reassuring and immediately put me at ease. He was quite lucid in his explanations of what had to be done, etcetera, and is making good progress with the ongoing treatment. Also, the rest of the staff were friendly and just plain nice.

John Jacobs

Wow wow Dr Claudia is the best dentist in London, my kid adores her and it is always a pleasant visit. Can’t recommend her enough

Bel Cohn

I am delighted with the results of my replacement veneers.

Dr Benedict Harrison was very good at talking me through the whole process and keeping me informed during the treatment if there were any issues - which there weren’t.

He has a charming demeanour which is an added advantage in a dentist, who I think we all agree are not our favourite people, overall a 5 star experience.

Karen Shepherd

I went to the Maida Smiles Clinic for the first time and had the most wonderful experience! I am dentist phobic but strangely left the place with a big smile on my face (literally!). The receptionist was super kind, Susan was impeccable and very helpful, and I had my appointment with the most amazing dentist Dr Mafalda Vilhena. Couldn’t recommend her enough. She explains the whole procedure and is very rigorous in her work. Congratulations to Dr Pedro Gutierres for creating such an excellent working environment and to all the team. I will be back and bringing my family along and we don’t even live in Maida Vale. Thank you for such an impeccable job on my teeth and for taking my fear of dentists away. Note: they don’t cover NHS, only private bookings or with insurance.
