Dry socket is a painful condition that can follow a tooth extraction, most often a wisdom tooth. After a tooth comes out, a blood clot forms in the empty socket and protects the bone and nerve endings underneath while the gum heals. In dry socket that clot does not form properly or is lost early, so the exposed area becomes sore, and pain that was settling can start to build again a few days after the extraction.
It is not common, and it does get better with treatment from a dentist. Knowing what normal recovery feels like, and what does not fit it, helps you decide quickly whether to ring your dentist. This post covers the symptoms and timing, who is at higher risk, how to lower the chance, what treatment involves, and the signs that need urgent help. Your own dentist's written instructions always come first, because advice differs a little between clinics.
Dry socket is the common name for a socket that has lost, or failed to form, its protective blood clot. According to the NHS, it is a painful condition where the blood clot over the tooth socket does not form properly or is dislodged. Dentists sometimes call it alveolar osteitis, meaning inflammation of the socket bone.
The clot matters because it covers the bone and nerve endings in the socket. Without it, the exposed bone can be very sore, and food and fluid can irritate it. The American Dental Association describes it as happening when the clot is displaced, leaving bone and nerves exposed.
Gloucestershire Hospitals NHS Foundation Trust says dry socket is not technically an infection but is due to the loss of the blood clot from the socket. Hull University Teaching Hospitals NHS Trust describes it differently, as a local infection linked to food collecting in the socket. Leaflets do vary in how they explain it, but they agree on the main points: it is painful, it follows an extraction, and a dentist can treat it.
The main sign is pain that is more than you expect, or pain that comes back after it had started to settle. Gloucestershire Hospitals says severe pain is common with dry socket. Other signs commonly described include:
Different people notice different things, and these signs can also have other causes, such as an infection. Only a dentist can tell which it is.
Most descriptions place the onset a few days after the extraction. Gloucestershire Hospitals says it usually happens a few days after the tooth has been removed, and that it is not common. The Bedfordshire Hospitals NHS Foundation Trust leaflet says symptoms usually start in the first 24 to 48 hours. Hull University Teaching Hospitals says pain can return 3 to 10 days after surgery.
The range is wide because leaflets describe slightly different groups of patients, and every mouth is different. The practical point is that if pain is getting worse instead of better in the days after an extraction, it is worth a call.
After a wisdom tooth removal, some soreness, swelling and oozing are expected. The NHS says you can usually go back to normal activities the day after, that pain and swelling should start to improve after one or two days, and that symptoms can last up to two weeks. If you had stitches, they usually dissolve by themselves. Our post on whether PRP is necessary for wisdom teeth removal looks at one way healing can be supported.
| What you notice | Usually part of normal recovery | Worth calling your dentist about |
|---|---|---|
| Pain | Soreness that is easing day by day | Pain that gets worse after a few days, or that painkillers do not ease |
| Bleeding | A little oozing for the first day or so | Bleeding that does not stop with firm pressure |
| Swelling | Swelling that peaks and then improves | Swelling that keeps growing or feels hard and hot |
| Taste and smell | A mild taste of blood early on | A persistent bad taste or smell with pain |
| Temperature | Feeling tired after surgery | A high temperature or feeling unwell |
This is a guide, not a diagnosis. If something feels wrong to you, trust that and ring your dentist.
No one can say who will get dry socket, but several things are linked with a higher chance. According to Gloucestershire Hospitals NHS Foundation Trust, dry socket is a little more common in smokers. The Bedfordshire Hospitals leaflet says the risk is higher for people who smoke or vape, or who have had prolonged or complex extractions.
Being in one of these groups does not mean you will get dry socket, and not being in one does not rule it out.
Prevention comes down to protecting the clot while it settles. The advice below is drawn from NHS hospital leaflets, and your dentist may give you slightly different timings. Follow their written instructions if they differ.
None of this removes the chance altogether, but it lowers the likelihood that the clot is disturbed.
Dry socket needs a dentist, not just home remedies. The American Dental Association describes treatment as cleaning the site of the extraction and placing a medicated dressing in the socket, which may be changed until the pain eases and the socket begins to heal. The Bedfordshire Hospitals leaflet lists regular suitable painkillers, gentle mouthwashes and a soothing agent placed in the socket, and says antibiotics are usually not required. Hull University Teaching Hospitals says treatment involves washing out the socket and sometimes placing a dressing.
At an appointment, your dentist will look at the socket and ask about your pain and its timing. The dressing is usually placed on the same visit, and you may be asked to return for review.
Between visits, follow the pain relief advice on the packaging or from your pharmacist or dentist, keep to gentle rinsing, and avoid smoking. Your dentist will tell you how long healing might take, which varies from person to person.
Pain after an extraction can also come from an infection, from a sharp piece of bone, or from the usual healing process. This is one reason to have it checked rather than guess. Our emergency appointment lets a dentist assess the pain, take an X ray if it is needed and treat it on the same day where possible.
The NHS describes wisdom tooth removal as usually a simple procedure, though as with any procedure complications can happen. Most people have a local anaesthetic to numb the gum, and sedation or a general anaesthetic is generally reserved for harder removals or for anxious patients.
The first day or two is usually the sorest. The NHS says swelling and pain should start to improve after one or two days, though symptoms can last up to two weeks. Plan a quiet day, eat soft foods, and take any time off that your dentist suggests. After sedation, the NHS advises avoiding driving for 24 hours after a sedative injection and 48 hours after a general anaesthetic.
If you are nervous about having a tooth out, ask about options in advance. We offer IV sedation as part of our dental sedation options, and our support for dental anxiety describes the other ways we help nervous patients. Our oral surgery team handles wisdom teeth and other extractions, and our guide to tooth extraction explains the difference between a regular and a surgical removal.
Surgical removal is more common when a wisdom tooth is impacted, meaning it cannot come through fully. Our information on impacted wisdom teeth explains when this causes pain and swelling that needs attention. The fees for extractions are on our extraction fees list, and the dentist will give you the fee for your case after assessment.
Ring your dentist, or ask for an urgent appointment, if pain is getting worse a few days after an extraction, or if the pain in the socket is severe. Our dentists can see emergencies the same day, and the emergency dentist service explains what the appointment includes.
The NHS advises seeking urgent dental advice, or contacting NHS 111, if you have bleeding that will not stop, severe or worsening pain and swelling that painkillers do not ease, or pain with a bad taste, a high temperature or feeling unwell.
Call 999 or go to A&E if you are finding it hard to breathe, speak or swallow, or have a lot of swelling in your mouth, as the NHS advises for a dental abscess. Do not drive yourself, and ask someone to take you or call an ambulance. Heavy bleeding that you cannot control with firm pressure also needs urgent medical help.
If you are due to have a tooth out, a short conversation beforehand about smoking, medicines, aftercare and who to call can prevent a lot of worry. If you have had one out recently and the pain is not settling, an appointment is a better option than waiting. Bring details of when the tooth was removed and what you have taken for the pain.
It varies. With treatment from a dentist, the pain usually eases much sooner than if left alone, and the socket then heals over the following days and weeks. Your dentist can give you a realistic idea after seeing it, because the timing depends on the person and the extraction.
Typical signs are pain that gets worse a few days after an extraction rather than better, a bad taste, and a very sore socket. These signs can have other causes too, so only a dentist can say for certain.
Not technically, according to Gloucestershire Hospitals NHS Foundation Trust, which says it is due to the loss of the blood clot. Other leaflets describe it differently, and an infection can occur separately. Antibiotics are usually not required for dry socket on its own, according to the Bedfordshire Hospitals leaflet, but your dentist decides.
It may settle over time, but it is usually very painful in the meantime. Dentists treat it by cleaning the socket and placing a dressing, which is why a visit is worth it rather than waiting.
Protect the clot. Avoid smoking and vaping, do not rinse vigorously in the first 24 hours, leave the socket alone, and eat soft food. Follow your dentist's written aftercare, because the exact timings vary between clinics.
Smoking and vaping are linked with a higher risk, according to NHS hospital leaflets, including Gloucestershire Hospitals and Bedfordshire Hospitals. That does not mean every smoker will get it, but it is the factor most often named, so your dentist will ask you to avoid smoking around your extraction.
Yes. It is most often talked about after wisdom teeth, but it can follow any extraction. The same aftercare applies, and the same warning signs are worth acting on.
Get advice if pain is getting worse rather than easing after a few days, if bleeding will not stop, or if you have a high temperature, spreading swelling or a bad taste. Call 999 or go to A&E if swelling affects your breathing or swallowing.

Being very nervous about using dentists after a terrible experience when I was younger, I came to Maida Smiles and loved the calm patient demeanour of Pedro. You can clearly see Pedro and the team take pride in what they do and are perfectionists. The whole experience was great and I will never go to another dentist again.

Delighted with the outcome of my treatment by Pedro. Meticulous attention to detail explaining exactly how the procedure would be carried out from first consultation to completion backed up with supporting guidance notes. Clearly an expert in his field and someone who cares passionately about the outcomes for his patients. Highly recommended.

I loved my first visit to Maida Smile Dental Clinic. The staff member at reception was very warm, welcoming and professional. My dentist, Priscilla, and her assistant were fantastic. I have quite sensitive teeth near my gums and I can find the probing and investigation quite uncomfortable during a consultation, but this wasn’t the case at all. I didn’t experience discomfort and felt in very safe hands. This also applied to the hygienist work. I didn’t even have to ask Priscilla to avoid using the ultra sound cleaner on the sensitive areas of my teeth. I’m looking forward to going back, and that’s not often you can say that about going to the dentist!

Since I first had the luck to be recommended and book an appointment with Dr. Pedro Gutierres over 8 years ago, I have never trusted my dental health nor my family’s to anyone else. It’s very hard to find a dentist who ticks all the boxes and even goes beyond any expectations you may have, from service, sympathy, professionalism, and expertise, etc., 5 stars are not enough to rate him. I was so happy when he opened his own clinic and surrounded himself with the best professionals you can find, I cannot recommend Maida Smiles Dental Clinic enough.

“Pedro was so patient and professional throughout my treatment for my implants.. I chose to have my treatment with a local anaesthetic rather than any sedation and have a very bad gag reflex when anything touches the back of my throat. His patience throughout the experience was wonderful, completely putting me at ease and he also gave me a technique to help me in the future. I have seen him for my follow up and fully intend to book an annual visit in 2020. which is no small thing as I live 60 miles away.I can thoroughly recommend him for any treatment.”

I was very nervous about having a dental implant but was immediately put at ease by Dr Pedro Gutierres. The procedure was explained in full and the care both during and after treatment has been of the highest standard. I received advice on how to look after my implant and have had no problems. I highly recommend.

I had 7 implants installed by Dr Pedro Gutierres, including one which required a bone graft. In my 70 years I’ve had some bad experiences with dental treatment so it takes a lot for me to relax but, from my initial consultation, through to the end of the treatment, I had complete trust in Pedro’s expertise: preparations for the treatment and after-care were extremely thorough and fully explained, the treatment itself was surprisingly endurable (considering what was involved) and the implants have made a major difference to my life. If you are considering implants then I would whole-heartedly and sincerely recommend Pedro for this work.

I went to different clinics before, and nobody could really convince me until Maida Smiles did. The team here makes you comfortable , shows you different options and different budgets , and doesn't try to make you go only for one plan. I felt that the team was knowledgeable and wanted to help me , so that's why I decided to start

I feel like I've not just developed a professional relationship with my dentist and the staff... they become more like friends as well, because of the kind of compassion and sympathy that they give you during treatment.

I am always anxious when having to see the dentist and this was one of my first experiences with Maida Smiles. The dentist, Benedict Harrison was very reassuring and immediately put me at ease. He was quite lucid in his explanations of what had to be done, etcetera, and is making good progress with the ongoing treatment. Also, the rest of the staff were friendly and just plain nice.

Wow wow Dr Claudia is the best dentist in London, my kid adores her and it is always a pleasant visit. Can’t recommend her enough
Bel Cohn
I am delighted with the results of my replacement veneers.
Dr Benedict Harrison was very good at talking me through the whole process and keeping me informed during the treatment if there were any issues - which there weren’t.
He has a charming demeanour which is an added advantage in a dentist, who I think we all agree are not our favourite people, overall a 5 star experience.
Karen Shepherd
I went to the Maida Smiles Clinic for the first time and had the most wonderful experience! I am dentist phobic but strangely left the place with a big smile on my face (literally!). The receptionist was super kind, Susan was impeccable and very helpful, and I had my appointment with the most amazing dentist Dr Mafalda Vilhena. Couldn’t recommend her enough. She explains the whole procedure and is very rigorous in her work. Congratulations to Dr Pedro Gutierres for creating such an excellent working environment and to all the team. I will be back and bringing my family along and we don’t even live in Maida Vale. Thank you for such an impeccable job on my teeth and for taking my fear of dentists away. Note: they don’t cover NHS, only private bookings or with insurance.
